Instruction to Students
Click on your appropriate batch link carefully. Ensure the Google form opens and verify the title matches your batch.
Enter your email and name. Name will be kept anonymous by the Principal. Click next button to go to the next section.
Each section is relative to one particular course(subject) and teacher concerned. Based on the questions (read carefully) choose your options form 1(very Poor),2,3,4, 0r 5(Excellent). All choices are compulsory to be answered. You may fill your issue faced and suggestions for improvements optionally.
Take time, answer all questions carefully and provide your feedback correctly to the extent possible. It should not take 7-10 mins of your time. Every student on current Roll must submit their forms individually. Do not consult with your friends. Your feedback is personally yours.
Go to Next section (see progress bar) until the last section where you press the submit button. Once submitted successfully wait for the confirmation message.
Those who fail to submit their forms will have their semester results withheld. Submit by 13th DEC 2019 (Saturday 4 pm) POSITIVELY.
